It is a person dressed in a bear costume that has a large opening that exposes the entire rear end of the person in the costume. It appears that the person is performing a sexual act on a man. The person in the bear costume could be a man or woman, but whatever is happening there is clearly perverse. The point is to show the audience that the manifestations of the horrors that occurred in that hotel are real and are not hallucinations that exist in the mind of the caretaker Jack. We, the audience, now know that Wendy knows that the hotel is "haunted". It's not that deep, anon.If the person in the bear costume and the man on the bed were real, most people would expect them to be ashamed of being caught in the act of doing something perverse, and they would quickly close the door or cover themselves with the sheets or run to the bathroom and close the door. Most people are not going to think "Oh, there's an opening in the prison wall and those two people are going to run at me and attack me". The two people make no motion as though they feel a sense of fight or flight. It's as though it is a little 10-second vignette meant to play over and over just to scare Wendy, just like a mischievous person who yells "Boo!!" and then instantly giggles because they induced a harmless fright in someone. None of this is deep, anon. You are of course welcome to dig into it and make of it whatever you like but it's just an attempt by a really talented and creative director to make a scary movie.
