He got attacked helping Talia escape the prison pit, somehow he ended up losing part of his lips/nose leaving him on constant agony so he apparently needs a steady stream of magic anesthesia to not be in seering pain at all times even though that's not how such injuries work but whatever. Whole thing was so stupid they removed most of the backstory from the movie and the bit about the gas mask was only confirmed by the costume designer but you can see hints of it in the movie.
